Care Needs Review & Care Planning

An RN-led review to help families understand care needs, risks and suitable support

Where helpful, an RN can review daily routines, health changes, medicines, mobility, falls risks and family concerns, then recommend practical next steps. When an older parent is no longer coping at home, the review can help separate urgent concerns from care-planning needs. Initial care direction is free; any in-home clinical assessment or written care plan is discussed and agreed before it begins.

Specialised support for complex care needs, comfort, dignity and family communication

Jade Care can support families managing complex health needs, serious illness, palliative comfort care or end-of-life care at home where needs can be safely supported and appropriate clinical oversight is in place. We focus on comfort, safety, carer relief, family communication and coordination with the person’s GP, specialist, palliative care or hospital team.
