Make sure the assessment sees the whole picture.
A My Aged Care assessment should reflect how life is actually being managed at home—not only the answer given in one moment. We help older people and families organise a clear, factual picture of daily needs, safety concerns and the informal support already holding things together.
Our role—and its limits
Jade Care provides independent, practical preparation guidance. We can help you describe a typical week, organise observations, identify existing support, and prepare questions and relevant documents.
We are not My Aged Care or an assessment organisation. We do not conduct the government assessment, decide eligibility, influence the assessor, or guarantee a service, classification, funding amount or start date. This page is general information only—not legal, financial, diagnostic or emergency advice. Content reviewed 25 July 2026 by Qixuan Dong, Registered Nurse (NMW0001813915).
Illustrative situations families may recognise
-
An older person may say that a daughter “only helps occasionally”, while she actually visits most days to prepare meals, organise medications, supervise showers and manage appointments. A typical week may show support that a polite answer hides.
-
A fall may sound isolated. The family may also have noticed increasing unsteadiness, difficulty getting out of bed, or reluctance to shower without someone nearby. The pattern gives useful context; it does not guarantee any assessment outcome.
-
Practical observations may include missed medications, unpaid bills, repeated calls, spoiled food or unsafe cooking. These details do not establish a diagnosis, but they may help the assessor understand whether further assessment or support should be considered.

We can help you describe a typical week, organise observations about daily tasks and safety, identify unpaid support already being provided, and prepare questions and relevant documents. You decide what information to share.

No. Jade Care is independent from My Aged Care and assessment organisations. We cannot decide eligibility, influence the assessor, guarantee a classification or budget, or promise when funded services will begin.
