Care led by experience, centred on families.
When a parent needs more support, families often need more than a list of services. They need someone who can listen, explain what may be changing, and help them consider the next step with calm and confidence.
Jade Care is guided by RN experience and a family-focused approach to care at home. We focus on dignity, comfort, safety and clear communication with the people who matter most.
Questions families often ask
Not sure how to start the conversation?
Start a care conversation with Jade Care. We can help your family talk through what has changed, what support may help, and what the next step could look like.
-
This is very common. Many older people minimise what is happening because they want to stay independent, do not want to worry their family, or feel uncomfortable accepting help. A calm first step is to write down what has changed — falls, missed meals, medication concerns, personal care difficulties, memory changes or carer stress — and then discuss what support may help.
-
Older people may unintentionally understate their needs during assessment. With their agreement, family members can help provide clear and honest examples of what happens day to day. Jade Care can help families prepare these examples so important needs are not missed.
-
It may be time to explore support when daily tasks are becoming harder, a parent has had a fall, personal care is becoming difficult, medication routines are confusing, or family carers are feeling overwhelmed. You do not need to wait until things become urgent before asking what help may be available.
-
Yes. We can help your family understand what has changed, what questions to prepare, what support may be needed at home, and how to approach the My Aged Care process. We do not decide eligibility or funding, but we can help you feel more prepared.
-
In some situations, families consider private support while My Aged Care assessment, funding or provider arrangements are still being organised. Jade Care can discuss what practical help may be suitable, such as personal support, medication-related support, post-hospital support, carer relief or check-ins at home.
-
We offer flexible pricing based on project type and complexity. After an initial conversation, we’ll provide a transparent quote with no hidden costs.Support may range from everyday living assistance, personal care, meals, transport and companionship to medication and nursing support, post-hospital needs, complex care, PEG feeding, tracheostomy care and palliative support. The right support depends on the person’s needs, clinical instructions, staff availability and safety considerations.
-
No. Jade Care is not My Aged Care and does not decide eligibility, funding or approved services. We help families understand the process, prepare questions, consider practical care needs and discuss possible support at home.
