About Jade Care

Jade Care was created to support older people and families when care at home starts to feel more complex.

For many families, the need for support does not arrive all at once. It may begin with small changes — meals being missed, medication becoming confusing, personal care becoming harder, a fall, a hospital stay, or a family carer feeling exhausted.

At that point, families often need more than a service list. They need clear guidance, practical care options and someone who understands both the clinical and emotional side of care at home.

Our approach is calm, practical and family-centred.

We take time to understand what is changing, what support may be needed, and what matters most to the older person and their family. We believe good care should respect routines, preferences, dignity, culture and the person’s sense of independence.

Whether a family is exploring everyday home support, nursing care, complex care or palliative support, Jade Care aims to make the conversation clearer and less overwhelming.

Jade Care helps families explore practical support at home, including everyday living support, medication and nursing support, post-hospital care, complex nursing care, palliative support, carer relief and My Aged Care preparation conversations.

Some services depend on clinical instructions, staff qualifications, risk assessment, equipment needs and availability.

My Aged Care boundary

Jade Care is not My Aged Care and does not decide eligibility, funding or approved government-subsidised services.

We can help families understand the aged care process, prepare clear and honest examples before an assessment, consider practical care needs and discuss possible support at home.

Eligibility and funding decisions are made through the official My Aged Care assessment process.
